Why is Population Health Management Important to Employers?

Population health management (PHM) is a concept that is spreading rapidly, particularly in the healthcare sector. It refers to strategic efforts to improve the health outcomes of individuals who are part of a larger group, whether that group is a company’s employees or members of a community.
